| To schedule an appointment
with our office we suggest you call our office between 9:30am and 4:00pm
Monday thru Friday. Our scheduling staff will ask you several important
questions when they schedule your appointment. It is very important that
we are aware of which insurance you carry before you come into our
office. Many insurances require you to obtain a referral from your
primary care physician. Some of the testing we do in our office also
requires a pre authorization from your insurance company. This must be
acquired prior to your visit with us. Patients are responsible to
contact their primary care physician for referrals prior to their visit. |

| If you are
referred to our office from your primary care physician, please provide
us with as much medical information as possible, including previous
testing. This will help us evaluate your condition. |

| If you must
cancel an appointment, let us know at least 24 hours in advance. |
